Child or Medical Identification Card

Be prepared in case of an emergency, create identification cards for your children, yourself or an older relative. Have their medical information close at hand.

You can easily design a card with more then business in mind. Create a card with your personal, medical, or your child's information including a photo. Any office supply store will have different papers to print on. For this article use Avery #5371. They tear apart easily.

Side 1

  1. Open Microsoft Word, Click on Start, All Programs, Microsoft Office, Microsoft Word
  2. Change the font to Times New Roman, size 12 (use the Formatting Toolbar)
  3. Click on TOOLS, Letters and Mailings, Envelopes and Labels
    • Click on the "Labels Tab"
    • Click on "Options"
    • Click on "Product number 5371," click on OK
  4. In the Address box, press ENTER 1 time to put in a blank line
    • Type the first and last name, ENTER
    • Type your address, ENTER
    • Type your city, state and zip, ENTER
    • Type your phone number, ENTER, ENTER
  5. Click on "New Document"
  6. Save the document as "Identification Cards - Side 1"
  7. Add a picture
    • Click on INSERT, Picture, From File, click on the photo, click insert
  8. Format the picture
    • Click on the picture to turn in on
    • Click on FORMAT, Picture
    • Change the Format to in front of text (this allows you more freedom to place the photo)
    • Change the Size to about 1 inch (depends on your picture)
    • Move the picture to the right side of the card
  9. Click on Print Preview, make sure it looks correct
  10. Goto Side 2

Side 2

  1. Open Microsoft Word, Click on Start, All Programs, Microsoft Office, Microsoft Word
  2. Change the font to Times New Roman, size 12 (use the Formatting Toolbar)
  3. Click on TOOLS, Letters and Mailings, Envelopes and Labels
    • Click on the "Labels Tab"
    • Click on "Options"
    • Click on "Product number 5371," click on OK
  4. In the Address box, press ENTER 1 time to put in a blank line
    • Type: Height: (type the height), Type: Weight: (type the weight), Type: (the birth date, (00/00/00) ENTER
    • Type: Emergency Contact: (type their name) ENTER
    • Type: Emergency Phone: (type the number) ENTER
    • Type: Doctor: (type the doctor's name and phone)ENTER
    • Type: Allergies: (type any allergies) ENTER
    • Type: Medications: (type any medications) ENTER
  5. Click on "New Document"
  6. Save the document as "Identification Cards - Side 2"

To make the cards easier to read, select the title words, for example; Height, Weight, Emergency Contact, and change the formatting to bold, if this makes the words overflow, then adjust the size of the font to 11 or 10.

To protect the cards, many craft stores sell a clear plastic sheet that can be used to seal them. To seal, you place a sheet on the top and one on the bottom and press the plastic together with your fingers.
